Two Rivers Transition Program
Anoka-Hennepin is committed to providing special education services to students with disabilities who continue to require specialized instruction in order to address transition needs as determined by the Individualized Education Plan (IEP) team. Transition programming supports students with special needs who have completed high school but have not received their diploma and require specialized instruction in order to transition to living and working independently in the community.
Two Rivers Transition Program at Franklin is a secondary special education program designed for students with educational disabilities and transition needs who are between the ages of 18 and 22 in post-secondary education and training, employment, and independent living. The program offers a functional and vocational curriculum with a focus on the acquisition of skills to promote increased independence in the community and a successful transition from high school to adult life. We strive to provide experiences and learning that will provide the foundation for students to have meaningful and fulfilling adult lives as individuals, family members, employees and citizens.
Program components include:

- Classroom instruction in transition areas (postsecondary education and training, employment, independent living)
- Hands-on learning opportunities

Student hours: 7:35 a.m. - 1:35 p.m.
Daily class schedule
Students participate in instruction, job training skills, obtaining employment and independent living activities which varies by student needs and schedule. Classes and activities led by school staff are held at Two Rivers Transition Program at Franklin, as well as in a variety of community locations. Community sites include work and volunteer experiences that are built into student schedules for a full trimester or year at a time.
Homeroom: 7:25 a.m. - 7:50 a.m.
Period 1: 7:55 a.m. - 8:50 a.m.
Period 2: 8:55 a.m. - 9:50 a.m.
Period 3: 9:55 a.m. - 10:50 a.m.
Period 4: 10:55 a.m. - 12:25 p.m.

Positive Behavioral Interventions and Supports (PBIS)
Positive Behavioral Interventions and Supports (PBIS) will provide an evidenced based proactive and positive educational environment in which all those who enter feel a sense of belonging and community. The Bridges and Pathways Programs will be a place where students are able to learn and achieve because the expectations of safe, independent, kind, and respectful behavior will be taught and then reinforced through the use of clear and common language by all staff.
Two Rivers Transition Program will:
- Teach and reinforce positive school behavior to follow school expectations
- Create a predictable environment and use consistent language, positive interventions, and predictable consequences
- Promote a positive school climate with shared responsibility among all staff
- Develop the relationship among our school, students, families, and community
- Promote a safe environment where kind and respectful students become independent adults
