Elementary families: Select Trimester 2 learning format preferences for each child attending an Anoka-Hennepin elementary school by Oct. 26

Anoka-Hennepin Schools is collecting learning format preferences from parents and guardians of elementary students for Trimester 2 with classes beginning on Tuesday, Dec. 8.

Please share your family’s intentions for your student(s) through the questions on this form by Monday, Oct. 26. A note, a message like this has been sent to parents/guardians for each elementary school-age child attending an Anoka-Hennepin school since specific information is needed for each child enrolled. Please make sure to submit one response for each child and note that only one parent/guardian needs to complete the form for each child.

Fill out this form.

Translated forms: Spanish, Hmong, Vietnamese, Russian, Somali and Arabic. Please print out, complete the form, and return to the school office where your student attends or contact the school for other options to provide your preference.

Information and answers to frequently asked questions are at ahschools.us/COVID-19. A comprehensive “Return to School Roadmap” provides an overview of school learning formats at ahschools.us/COVID-19guidebook. Translated materials are available in Spanish, Hmong, Vietnamese, Russian, Somali, and Arabic on the district “Return to School” website.

Parents/guardians of secondary students will also receive a communication that informs how they can select a learning format for their student(s) for Trimester 2.

Should the district not receive a response from you, your student will remain in their current learning format. The district may be required to change learning formats at any time to comply with state guidance. Thank you for taking the time to respond and your ongoing support of your student and our schools.
