(03/02/2023) The 2022 1095c forms have been generated and are available for employees to download through Skyward employee access. Any employee that did not complete the submission request to receive the form electronically will receive their form in the U.S. mail this week.

1. Click on the Lock and Key, enter your username and password.
2. Select the Skyward icon and go to Employee Access.
3. Click on the Printed 1095 Forms icon within Employee Access.
4. Under the 1095 Forms heading, click the icon next to the 1095 year you wish to print a 1095 form for.
5. Select one of the options to print your 1095 form from the menu.
   1. Print Full SSN 1095: Selecting this option generates a PDF copy of the 1095 form that includes your social security number.
   2. Print Masked SSN 1095: Selecting this option generates a PDF copy of the 1095 form that masks the first five digits of your Social Security Number.
6. Print the PDF.
