Back-to-School Forms

Please complete back-to-school forms by Sept. 27

Even with school underway, there are still a fair number of parents/guardians who haven’t completed annual back-to-school forms for their student(s).

Safety is the most important reason the district asks for completed back-to-school forms. Gathering this information gives schools the ability to reach parents/guardians if an issue or emergency were to present itself, and more successfully attend to health matters.

This year, it’s easier than ever to get started with the back-to-school forms process – it’s completely online.

Go to the My Student Information icon via the A-HConnect system (accessible via the lock and key icon at the top right of district and school websites) and let the prompts guide you from there. Step-by-step instructions are also available at ahschools.us/forms.

My Student Information also holds a variety of student information, including attendance as well as progress reports and grades at the middle and high school levels.
