Waiving your health, dental and/or life insurance coverage? If YES, you must attend/join an Orientation/Benefits meeting and waive your options in smartben, our electronic enrollment system. You ALSO need to sign a decline/waiver form:
Decline/waive life insurance (part-time Teachers only)
All Teachers (benefit eligible) need to complete and turn in a Sick Leave Bank form (Accept OR Decline).
If you are electing to puchase Supplemental Life Insurance OVER $50,000 you need to complete a Evidance of Insurability (EOI) application ($50,000 or less your are automatically approved, upon enrollment).
