    Register and pay for activities online via A-HConnect/SchoolPay (coming soon). Click on the lock and key icon found toward the top of this page and select the activities registration icon. Please contact your school's activities office if you need help with online registration.

    Otherwise, you can fill out hard copies by downloading and printing the applicable forms below and submitting them, along with the fee, to the activities office of your high school. 

    Note: The eligibility bulletin is updated once each school year; the athletic physical form is updated every three years - contact your activities office for renewal status. The emergency care card is updated each season of participation. Contact your high school activities director if the activity fee could be a barrier to participation. Each school offers baseline concussion testing each year. Check with your child's activities director for more information. 
     

    Does my student need a sports physical for the 2020-21 school year?

    Yes, if: a student has not previously participated and does not have a medical clearance to participate, they'll be required to get a physical for the 2020-21 school year. Example: Freshman athletes that have not previously participated in high school athletics will need a physical to participate this school year. 

     

    No, if: a student has a a physical on file that expires between March 12, 2020 and the conclusion of the 2020-21 school year will have clearance extended through the completion of the 2020-21 school year. Example: Senior athletes usually need a new physical prior to senior year because after three years they're invalid. Under this new rule, however, they won't expire until the end of the 2020-21 school year.

     

    Note: every athletic participant needs to complete the annual health history questionnaire. 

     

    Optional insurance

    Parents can purchase optional insurance via 1st Agency using a Visa or Mastercard. Select Minnesota as the state and Anoka-Hennepin as the school district. Read the letter that was mailed to parents.

     

    Academic eligibility

    The complete language for academic eligibility is included on the eligibility bulletin. Here is a summary of credit totals that must be completed prior to term/trimester in Anoka-Hennepin.
